Request for Application (RFA) for the Mississippi Delta Health Collaborative Clinical Community Health Workers Initiative

The Mississippi Department of Health seeks applications from healthcare systems to implement the Clinical Community Health Workers Initiative across 18 specified Delta counties, focusing on primary care improvements for detecting, managing, and treating hypertension and high cholesterol disparities. Systems will hire and deploy Community Health Workers as patient liaisons to deliver self-management education, care coordination, and community outreach for caseloads of 100 patients each. Up to 15 awards of as much as $140,000 per CHW are available through September 2029 for qualified systems that form interprofessional teams and meet outcome-based deliverables.
